The undersigned, understand and agree with the following:
The Dr. Ornish’s Program for Reversing Heart Disease® program manuals, resources, and other information (“Materials”) can be accessed on a private portal at www.ornish.com. The Materials are proprietary and confidential.
The Materials may only be used as specified in a program license agreement to deliver the Dr. Ornish’s Program for Reversing Heart Disease® at a program site that has been trained and certified by Dr. Dean Ornish. Reproduction or transmittal of the Materials in any form or by any means, electronic or mechanical, including photocopying, recording, or by any information storage or retrieval system, is prohibited except with the prior written permission of Dr. Dean Ornish.
